Meeting Registration


+++IMPORTANT+++ To qualify for one of the (Regular, Associate) member registration rates, paid 2019/2020 membership in that category is required at the time of the meeting


+++IMPORTANT+++ If you register for one of the "local" registration rates, proof of local home address must be presented when you pick up your badge at the conference


INSTRUCTIONS:

Please fill out the form below

  • enter the number 1 into the appropriate member category (separate registration for each attendee; no multiple registrations under one name); click here for more details on the registration categories

  • enter the number of accompanying adults and name(s) (if applicable), which pays for participation at the Welcome reception and Banquet dinner

  • enter the number of persons attending the Welcome reception and Banquet dinner

  • click "Submit"

  • this will redirect you to Paypal where you must pay via a Paypal account (if this is not an option available to you, please read on)

AOPT is using the secure and convenient payment processing offered by PayPal. If you do not yet have an account, you must create one and pay with a valid credit card (or bank account where offered).

PayPal is accepted by thousands of merchants around the world. The secure payment processing through Paypal allows payment with a valid Visa, Mastercard/Eurocard, Discover or American Express credit card. In certain countries you may have additional card options. Why use a PayPal account?

Some companies or institutions do not allow the association of a company or institutional credit card with a Paypal account. In this case, you may register using one of the following options:

  1. Register online here and pay via a personal Paypal account (with personal credit card) and request reimbursement from your company or institution
  2. Register and pay onsite at the AOPT 2019 conference when checking in at the registration table
    a. AOPT is able to accept payments via credit card when the card is present
    b. with a check made out to AOPT (check drawn on a US bank and made out in US Dollars only)
  3. Register by mail and pay with a check at least 3 weeks BEFORE the conference (check drawn on a US bank and made out in US Dollars only). Please send your request to the Treasurer (treasurer@aopt.org) to receive mailing instructions.

If selecting one of the options #2 or #3, complete the form below but do not click the "Submit" button. Instead, print the webpage with the completed form and submit it along with your registration fee. Then, exit this form without clicking the "Submit" button.

If the request is received BY the early registration deadline of January 31st, you will be granted the early registration fee when paying at the conference. Otherwise, the late fee applies.
