Donation, Sponsorship, Exhibitor & Other Support

Thank you for supporting AOPT. The following types of donations can be submitted here by completing all required fields below.

• Contributing Member (fee of US $1000 or greater for 2019/2020 calendar years; check here to find out how you qualify)

• Corporate Sponsor (fee of US $5000 or greater for 2019/2020 calendar years)

• General Support of AOPT (please describe the purpose in the "Remarks" box)

• Young Investigator Travel Award (please describe the research topic/field in the "Remarks" box)

• Conference Exhibitor (please refer to the Exhibitor Prospectus for more information and sponsorship levels)

• Other Donation and Sponsorship (please describe the type of sponsorship in the "Remarks" box)

AOPT is a 501(c)(3) not-for-profit organization registered in the United States of America and your donation may be tax-deductible as applicable by law. Please consult with a tax advisor.

INSTRUCTIONS:

Please fill out the form below and click "Submit". All fields marked with a red asterisk must be completed. Additional information on your type of donation should be provided in the "Remarks" box, if applicable. If someone other than the Contributing member is making the payment on the member's behalf, please note the payer's name as it appears on the credit card in the "Remarks" box.

The "Submit" button will redirect you to Paypal where you must pay via a Paypal account (if this is not an option available to you, please read on).

AOPT is using the secure and convenient payment processing offered by PayPal. If you do not yet have an account, you must create one and pay with a valid credit card (or bank account where offered).

PayPal is accepted by thousands of merchants around the world. The secure payment processing through Paypal allows payment with a valid Visa, Mastercard/Eurocard, Discover or American Express credit card. In certain countries you may have additional card options. Why use a PayPal account?

Some companies or institutions do not allow the association of a company or institutional credit card with a Paypal account. In this case, you may make your donation by using one of the following options:

1. Fill out the form below and pay via a personal Paypal account (with personal credit card) and request reimbursement from your company or institution
   
2. Donate by mail and pay with a check (check drawn on a US bank and made out in US Dollars only). Please send your request to the treasurer@aopt.org to receive mailing instructions.

If selecting option #2, complete the form below but do not click the "Submit" button. Instead, print the webpage with the completed form and submit it along with your donation. Then, exit this form without clicking the "Submit" button.
